That's odd, but are you able to clone this server to perform an-place upgrade to see if it will resolve this issue?

I tried to do that, but the Windows 2016 64-bit English ISO that I downloaded from MS Evaluation Center wasn't identified as the correct version. Not sure why.
I still have the ISO that this VM was originally installed from, but was hesitant to use that since its so old.

What version did you use from the Evaluation Center, I suspect Essentials? As for the old ISO you can rebuild/slipstream the WIM-image and then recreate a new up-to-date ISO.

No, it wasn't Essentials. The file name downloaded is Windows_Server_2016_Datacenter_EVAL_en-us_14393_refresh.ISO.
If you can direct me to some instructions for how to create an up-to-date ISO from the original one, I'm very interested!

I've used this tutorial some time ago to recreate a new ISO file with the latest LCU/SSU.
