Step#1 - DISM /RestoreHealth Scan
Warning:this fix is specific to the user in this thread. No one else should follow these instructions as it may cause more harm than good. If you are after assistance, please start a thread of your own.

  1. Right-click on the Start button and select Command Prompt (Admin)
  2. When command prompt opens, Copy (Ctrl+C) and Paste (Right-click > Paste) the following command into it, then press Enter
    Dism /Online /Cleanup-Image /RestoreHealth
  3. Once it finishes, copy and paste the following into the command-prompt window and press Enter. If prompted to overwrite the existing file go ahead.
    copy %windir%\logs\cbs\cbs.log "%userprofile%\Desktop\cbs.txt"
  4. Once this has completed please go to your Desktop and you will find CBS.txt => Please zip/upload to this thread.
    Please Note:: if the file is too big (over 7MB) to upload to your next post, please upload via a service such as Dropbox or One Drive or SendSpace and just provide the link.

Please do the following.

Step#1 - Capture Process Monitor Trace
 1. Download and run Process Monitor. Leave this running while you perform the next steps.
2. Attempt to install KB3186539 just like you have in the past.
3. Stop Process Monitor as soon as the update fails. You can simply do this by clicking the magnifying glass on the toolbar as shown below.

4. Select the File menu...Save... and save the file to your desktop. This is likely the default location. The name (unless changed) will be LogFile.PML. This is fine.
5. Zip up and attach the LogFile.PML file as well as your CBS.log.

Thank you. Can you navigate to your c:\windows\logs\cbs folder and also zip up and send me the two most recent logs (besides the current cbs.log)?

That was helpful. Thank you. Please try the following. You can turn off the automatic installation until we resolve your issue. Your choice.

Remove Update Manually
 1. Right-click on the Start button and select Command Prompt (Admin)
2. When command prompt opens, Copy (Ctrl+C) and Paste (Right-click > Paste) the following command into it, then press Enter
wusa /uninstall /KB:2894856

3. Let me know if it says it was successful or if there are any errors. If nothing appears to happen then something was likely typed incorrectly.

OK so I will assume you received a message box or something that came up and allowed you to uninstall it successfully....If that's the case then go ahead and attempt to install KB3186539 again and provide me the CBS.log if it fails.

Please do the following.

1. Click the Start button and type cmd.exe into the search box.
2. Right-click on cmd.exe that appears in the search results and select Run as administrator. Answer Yes to the UAC prompt that comes up.
3. Copy/Paste the followinig into the command-prompt window and hit enter.
wmic qfe get hotfixid | findstr /i /c:"KB3186539" >1 && notepad 1

4. Notepad will open after a few moments. Let me know what appears within the notepad file.

Ignore my previous post. In this case I wouldn't expect notepad to open so we are OK. Please check for Windows Updates again and provide a list of only the important updates that are listed.
