Upgrade from Windows 8.1 to Windows 10

Reboot and retry the upgrade from the ISO.

Tried yet failed again. Reached 29% and then went to "Undoing changes made to the system" or something along the lines then gave me the same SAFE_OS error at the end.

4. Press the Fix button just once and wait. If for some reason the tool needs a restart, please make sure you let the system restart normally. After that let the tool complete its run.
5. When finished FRST64 will generate a log on the Desktop (Fixlog.txt). Please post the contents of it in your reply.

Yep. Also, I forgot to mention. Every time I reboot my PC now has a Windows 8.1 Boot menu and a AVG Uninstaller menu. How can I get rid for the AVG Uninstaller one?

1. Press "Windows Key" and "R" to go to the Run command.
2. Please type msconfig and press "OK"
3. A Window will appear, please go to the "Boot" tab
4. Under the Boot Tab you will find your Operating System i.e Windows 8 and the AVG Anti virus Clear
5. Right Click on the AVG Anti Virus Clear and delete the file.
