Yes! That makes totally sense because SFC is complaining that it can't check the file!

I think this article Disable Narrator on Windows 8 Computer | Steps and Pictures explains how to block the narrator.exe only for the users, leaving SYSTEM and TrustedInstaller with no changes should not affect SFC and the system.

On the Narrator.exe file security properties is TrustedInstaller on the users list? If not delete c:\Windows\System32\Narrator.exe then run SFC scan it should copy the file and set the correct permissions.
 
Yes it is and it has full control.. I did not touch that..

Should I just delete it and let sfc /scannow replace it?
 
Actually yes, I had to delete it. I ran `sfc /scannow` again. Here's the results.

It pops up and pesters me when I press a seemingly random combination of buttons or by accident.

I have 1 update which HP - Enterprise WSD Multi-Function Printer, Other hardware - Null Fax - HP Deskjet 4640

It's basically my printer software. Whenever I try to install this update it fails. But I don't need it anyway, so I disabled it.

Now I performed another check for updates and it says "No updates are available".
 
Download Windows Repair (All-in-One) Portable
  • extract the tweaking.com_windows_repair_aio.zip to c:\Windows\TEMP the zip will extract to a folder called Tweaking.com - Windows Repair
  • execute the file Repair_Windows.exe from the Tweaking.com - Windows Repair folder, accept the Security warning and the EULA
  • click the button Jump To Repairs
  • accept the tool prompt and let the System Restart in Safe Mode to execute the next steps
  • In Safe Mode execute again the file Repair_Windows.exe from the Tweaking.com - Windows Repair folder
  • click the Open Repairs button to access the following screen:
    WindowsRepair.png
  • click the box All Repairs to uncheck all the box's
  • check only the following box's:
    • 12 - Repair Icons
  • check the box Restart/Shutdown System When Finished > Restart System
  • click the Start Repairs button

Note: If you see a prompt about a new version of the tool simply ignore that, we don't need the latest version for this fix.

I did everything you said.

Unfortunately the icon problem persists. The thumbnails look exactly as they did before (like here: Windows Explorer is not showing thumbnails and icons - Microsoft Community)

By the way I have also installed OldNewExplorer which makes the windows explorer in Win8.1 look more like Win7. I don't think this matters but just saying. I did uninstall it before to see if it was interfering and no.. icons/thumbnails were the same without it (i.e. broken).
