Failed to install Monthly Quality Rollup's since 07-2018 On Server's with SQL or Exch

Yes, when I try to install this update, I get this message:

"Security Update for Windows (KB3177467) is already installed on this computer."
 
No, I do not have PDQ Deploy installed on any of the servers with the problem.
 
Step#1 - FRST Scan

1. Please download Farbar Recovery Scan Tool and save it to your Desktop.
Note: You need to run the 64-bit Version so please ensure you download that one.
2. Right click to run as administrator. When the tool opens click Yes to disclaimer.
3. Please ensure you place a check mark in the Addition.txt check box at the bottom of the form before running (if not already).
4. Press Scan button.
5. It will produce a log called FRST.txt in the same directory the tool is run from (which should now be the desktop)
6. Please attach the log back here.
7. Another log (Addition.txt - also located in the same directory as FRST64.exe) will be generated Please also attach that along with the FRST.txt in your reply.
 
After checking for updates, installation and failure.
Note that this is a server without SQL, the only service remaining on it is SQL Server VSS Writer.

Can you please remove hardlock from the machine? There have been known issues with it in the past with regards to Windows Update.

Are there any other common denominators between the servers?
 
common denominators:
  • server 2008r2 x64
  • VM on 3 identical VMware ESXi 6.7.0 hosts
  • 3 Physical Servers (Host) HP ProLiant DL360 Gen9
  • SQL Server or Exchange
 
The problem also occurs on computers that do not have a hardlock driver installed
