Hello,
24278 seems to be associated with KB4486563 (another monthly security rollup) that seems
broken. Like KB4516065, WUSA shows KB4486563 not installed but a Search Registry shows
a lot of keys.

Hello,
Completed step 1.
Before firing off 4534310 again, I think we have one more KB in the same state - WUSA reporting uninstalled but FRST64 Search Registry shows hundreds of keys. I checked Security Monthly Quality Rollups from 10/2018 - 1/2020. This one, KB4489878, seems to be the last one, except there are well over a thousand keys in the registry for 4534310. Would they need to be cleaned up for a clean install?

Hello,
The fixlist is only for KB4534310.

What about KB4489878? The latest 4534310 failure log shows many entries for KB4489878 trying to load packages. Why? WUSA reports 4489878 not installed. 4534310 is a rollup so 4489878 shouldn't be a prereq. The searchreg file for 4489878 is 11K worth of stuff (junk?) in the registry. IMHO, nuking 4534310 without nuking 4489878 will be unproductive.

The fixlist is only for KB4534310.
Correct. I don't see a reference to the current problem and KB4489878 in the cbs.log yet so no need to remove it.

As always, it is your machine so you can decide to follow the given instructions or not.
I would let FRST64.exe process the given fixlist.txt.
