Upgrade to Windows 10 fails

Step 1:
Warning: This script was written specifically for this user, for use on that particular machine. Do not run this script on another machine.
  1. Download the attachment fixlist.txt and save it to your desktop.
  2. Right-click on FRST64.exe and select "Run as administrator".
  3. Press the Fix button.
  4. The tool will now process fixlist.txt.
  5. If for some reason the tool needs a restart, please make sure you let the system restart normally. After that let the tool complete its run.
  6. When finished, a log called Fixlog.txt will appear in the same directory the tool is run from.
  7. Attach the logfile Fixlog.txt to your next reply.

Step 2:
Double-click ComponentsScanner.exe again and click Yes on the UAC (User Account Control) prompt which appears.
Follow the on-screen instructions. If prompted to perform any repairs, press Y to agree.
Once complete, a report will be saved to your desktop called ComponentsScanner.txt.
Attach that file to your next reply.

Great. Try the upgrade again and report the result.
 
Re-ran the 22h2 Win 10 upgrade and it failed at the same point i.e. completes the installation of the files, reboots, starts on the "Working on the updates" but then around 8% - 13% complete it kicks up the "Something went wrong" message, reboots and rolls-back the changes. The error code & message are still the same.

The setup logs are attached. Let me know if you need any of the other files from the Panther folder.

Also confirming that the BSOD error, that appears during the "Working on updates" phase, is "IRQL not less or equal".
 
Is this issue still being looked at as there's been no messages in the past week ?

  • Click on Start - All programs - Accessories.
  • Rightclick on Command-prompt and select Run as administrator.
  • When command prompt opens, typ chkdsk into it, then press enter.
  • Wait for this to complete.
  • Download ListChkdskResult.exe by SleepyDude and save it on your desktop.
  • Rightclick

I also ran the Samsung Magician full diagnostic scan to detect and fix any issues with the SSD ... again no problems were reported.
 
Install failed again at the same place with the same BSOD "IRQL not less or equal" message.

So it goes through the "Installing" phase, PC reboots and the starts the "Working on updates" phase, got to 9% complete and then the BSOD kicked up with the "Your device encountered an error" message, the PC rebooted, tried to recover the installation and then rolled back the changes. On reboot the error was the same SAFE_OS during APPLY_IMAGE error.

  1. Click the Start button and in the search box, type Command Prompt
  2. When you see Command Prompt on the list, right-click on it and select Run as administrator.
  3. When command prompt opens, copy and paste the following command into it, then press enter.

    Code: 
    set > "%userprofile%\Desktop\set.txt"
  4. This will create a file, set.txt on your Desktop. Please attach that file to your next reply.
